What is BMI and Why Is It Still So Widely Used?

Body Mass Index (BMI) is a simple ratio of your weight to the square of your height, developed nearly two centuries ago by Belgian mathematician Adolphe Quetelet as a way to study average body proportions across large populations — not, notably, as a tool for assessing any single individual's health. Despite that origin, BMI has become the default screening measure used by doctors, insurers, and public health bodies worldwide, largely because it requires nothing more than a scale and a measuring tape, correlates reasonably well with body fat percentage across large populations, and is cheap and fast to calculate compared to more precise methods like DEXA scans or hydrostatic weighing.

The formula itself is straightforward: weight in kilograms divided by height in metres squared. A person weighing 70 kg who stands 1.75 m tall has a BMI of 70 ÷ (1.75 × 1.75), which works out to roughly 22.9 — comfortably within the standard "normal weight" range.

Standard BMI Categories

The World Health Organization defines four broad categories that most doctors and calculators still reference: below 18.5 is classified as underweight, 18.5-24.9 as normal weight, 25-29.9 as overweight, and 30 and above as obese, with obesity often further split into Class I, II, and III at higher thresholds. These cutoffs were derived from population-level studies correlating BMI with disease risk, and while they remain a useful starting point for a quick health screen, they were never intended as a precise diagnostic tool for any one person standing in front of a doctor.

Where BMI Falls Short

BMI's biggest limitation is that it can't distinguish between muscle mass and fat mass — both simply register as weight. A muscular athlete or a regular weightlifter can easily land in the "overweight" or even "obese" BMI category despite having low body fat and excellent metabolic health, purely because muscle is denser than fat. BMI Standards for Asian Populations

Research over the past two decades has shown that Asian populations, including Indians, tend to develop weight-related health risks like type 2 diabetes and cardiovascular disease at lower BMI values than the standard WHO cutoffs suggest, likely due to a tendency toward higher body fat percentage and different fat distribution patterns at the same BMI compared to Western populations. In response, many health bodies now recommend adjusted cutoffs for Asian populations: a BMI above 23 is considered overweight (rather than 25), and above 27.5 is considered obese (rather than 30). A BMI outside the "normal" range isn't an automatic health verdict, but it is a reasonable prompt to look closer. If your BMI reads as underweight, it's worth checking whether that's intentional and sustainable, or a sign of undereating, an underlying condition, or a metabolism running unusually fast — a doctor can help distinguish between these with basic blood work. If your BMI reads as overweight or obese, the more useful next step is usually looking at waist circumference (a rough proxy for visceral fat), recent trends in the number rather than a single snapshot, and how you actually feel — energy levels, sleep quality, joint stress — rather than fixating on the BMI number alone as the single measure of health.

Better Alternatives and Complements to BMI

Waist-to-height ratio, calculated by dividing waist circumference by height, is increasingly recommended alongside or instead of BMI, since it directly captures abdominal fat concentration — a ratio above 0.5 is commonly used as a simple flag for elevated health risk regardless of overall BMI. Body fat percentage, measured through methods ranging from basic skinfold calipers to more accurate DEXA scans or bioelectrical impedance scales, gives a direct read on how much of your weight is actually fat versus lean mass, solving BMI's biggest blind spot. And for a fuller picture, doctors often combine BMI with blood pressure, fasting blood sugar, and cholesterol panels, since metabolic health is genuinely multi-dimensional and no single number — BMI included — captures it completely.

BMI in Children and Teenagers

The standard adult BMI categories don't apply to children and adolescents, whose bodies are still developing and whose healthy weight ranges shift naturally with age and growth stage. For young people, doctors instead use age- and sex-specific BMI percentile charts, comparing a child's BMI against a reference population of the same age and gender rather than against a single fixed cutoff. A pediatrician is the right person to interpret a child's BMI reading — the calculators and categories most adults use, including this one, aren't designed for that purpose.

BMI as Part of a Bigger Picture

The most useful way to think about BMI is as one data point among several, not a verdict. Dividing by height alone would have made taller people appear systematically heavier relative to shorter people even at similar body proportions; squaring height corrects for this and is what allows BMI to be compared reasonably across people of different heights. BMI Changes Across Life Stages

What counts as a healthy BMI shifts somewhat across a person's life, even though the standard categories present a single fixed range. Older adults, for instance, sometimes benefit from a slightly higher BMI than the standard "normal" range suggests, since some research links very low BMI in older age with higher frailty risk and reduced ability to recover from illness — a nuance that a single BMI number and its category label doesn't capture.
